2. The following information for the cable locating device used for the detection is missing in the report :
  1. brand name; or
  2. model number; or
  3. serial number; or
  4. calibration record

3. Cable information as shown in the report is not accurate because :
  1. The deviation in cable alignment is more than 25% of the actual cable depth; or
  2. The deviation in cable depth is more than 25% of the actual cable depth; or
  3. There is deviation in cable quantity

4. The following underground cable information is missing in the report :
  1. alignment; or
  2. reference point; or
  3. depth profile; or
  4. cable quantity; or
  5. voltage level

5. Fail to mark the cable alignment or depth on site. 2
6. Plan of the underground cable obtained from the electricity supplier is not used. 3
7. The report details such as alignment, depth, etc. are not explained to the contractors, supervisors or workers before carrying out the excavation work. 3
8. Passive detection to locate cable alignment is not carried out. 3
9. Active detection (such as toroidal injection with a signal clamp) to identify cable position is not carried out. 3
10. Fail to identify or record the existence of cable. 3
11. Produce incomplete or inaccurate report leading to electrical accident which causes injury. 4
12. Produce incomplete or inaccurate report leading to interruption of supply to consumer or interruption to circuit by damaging cable of 11kV or below. 4
13. Produce incomplete or inaccurate report leading to electrical accident which causes death. 6
14. Produce incomplete or inaccurate report leading to interruption of supply to consumer or interruption to circuit by damaging cable of above 11kV. 6
